Union City Youth Sports Application

By registering my child for a Union City, NJ recreational program through the Union City Youth Sports application, I acknowledge and agree to the following:

1. Medical & Physical Fitness Acknowledgment

I certify that my child has recently been examined by a physician and, to the best of my knowledge, is physically fit to participate in the selected recreational program(s). I understand that participation in sports and physical activities involves inherent risks, including but not limited to sprains, fractures, concussions, or other injuries.

I voluntarily assume all risks associated with my child’s participation and agree that it is my responsibility to notify Union City, NJ’s Recreation Department of any medical conditions, allergies, or disabilities that may affect my child’s ability to safely participate.

2. Liability Release & Hold Harmless Agreement

I agree to hold harmless and release from liability Union City, NJ, its Recreation Department, employees, volunteers, coaches, and officials from any claims, damages, or injuries arising from my child’s participation in recreational activities, except in cases of gross negligence or willful misconduct.

I further waive any claims against individuals transporting my child to or from activities.

3. Photography & Media Release

I grant Union City, NJ permission to use photographs and/or videos of my child participating in recreational programs for promotional purposes. These materials may be used in newsletters, brochures, social media, television, flyers, and other public relations materials at the discretion of the Recreation Department.

I understand that:

● My child’s name may be included in publications submitted to local newspapers.
● If I do not consent to media use, I will submit a written request to the Recreation Department.

4. Code of Conduct Compliance

I acknowledge that all participants, parents, coaches, and spectators must adhere to the Union City, NJ Recreation Department’s Code of Conduct, as outlined in accordance with P.L. 2002, Chapter 74. Violations of the Code of Conduct may result in disciplinary action, including suspension or removal from programs.

5. Agreement & Acknowledgment

By completing my child’s registration, I confirm that:

✅ I have read and understood this Consent Waiver.
✅ I accept the risks involved in recreational activities and waive liability claims against Union City, NJ.
✅ I agree to abide by the Code of Conduct and all Recreation Department policies.
✅ I authorize the use of photographs/videos for promotional purposes unless otherwise stated in writing.
